I am looking for someone with ICS experience to help point me in the right direction.  I am looking for direct language that allows for or disallows the modification of standard ICS forms.  This will greatly help my department accept the forms and use them on a routine basis.

I know NIMS is designed so that everybody is on the same page.  I am not trying to reinvent the wheel here but it would be a lot easier to get my department to start using ICS forms if we are allowed to customize these for our use.  I have attached an image of the proposed form and a copy of the .doc file if anyone is interested.  Basically I would need to simplify the process of filling these forms out by preloading the information that I can.  We currently use a similar form that have criteria that better fits our unique need.  I have made every attempt to keep the form in the standard format so that is looks and feels like the standard ICS-214 but I have added check boxes so the IC can quickly acknowledge who was on the call.  I have also added incident type information in box 1 as our current form makes use of this information.

I would greatly appreciate the assistance of anyone familiar ICS and knows the ins and outs of the ability to modify these forms.

bmackie@mts.net if you are interested in seeing them. Keep in mind I am in Canada. Whether that makes a difference or not, I don't know what your requirements are.
I don't see anything wrong with using the form the way you have modified it. You haven't changed the structure, just prefilled in some things. We do the same thing with our version of NFIRS.
The forms are not nationally standardized. Many department's customize their forms, some even have them laminated or printed on dry erase boards. Check out my website. The key is to use the ICS system for greater firefighter safety.
You have already done the best you can with it. I recommend the Word docs because they're easier to modify without getting out of shape. You may run into some that are for Excel, but they're harder to modify. We're doing the same thing: changing the assignments and sections, etc., to fit our needs. We save everything and just reuse the same forms after modifying them. It's all good because we're starting with the same basic format. I don't know of any better way to do it.
For those that are interested I have been shown specific verbiage that allows the modification of ICS forms. According to National Incident Management System, December 2008 ed. page 129:

"The individual forms may be tailored to meet an agency’s needs. More importantly, even though the format is flexible, the form number and purpose of the specific type of form (e.g., Assignment List (ICS 204) defines the assignments for a Division or Group) must remain intact in order to maintain consistency and facilitate immediate identification and interoperability, and for ease of use."

So for our day to day operations, the modified form is sufficient.
