Colerain Township in Cincinnati lost 2 firefighters in a structure fire on April 4th, 2008. Captain Robin Broxterman and FF Brian Schira were performing an interior attack when conditions soured and a floor collapsed, sending them into the inferno below.

For details of this fire, you can now download a PDF of the preliminary report at www.coleraintwp.org

I am also uploading a copy of it here. I hope it works ok for you. STAY SAFE!!

This is also going to be (hopefully) discussed in the Building Construction and Firefighter Safety group:

That would be great. Robin's father expressed his wish that we all simply learn something from her death.
No. They didn't do a 360 - The front of the structure (as you can see from the photo) gives no real indication of a walk out basement. Had they known, I'm also sure they would have chosen that rather than the interior stairwell.

It was dark and drizzling when they pulled up and they were already behind the 8 ball on time after missing the driveway and having to back up 740 feet. They just made entry thru the front door like most of us would have. Colerain has since changed their SOP for basement fires to include a 360 prior to entry.
