The Litter is called a Stokes Basket and is used to carry victims. The wheel is normaly attached to the bottom and aids the rescures to move it over rugged ground long distances it can be removed easily.

In this picture it is in side a wooden replica of a UH60 Helicopter Cabin which is to scale . The wheel is placed on top so to allow the stokes to fit in the helicopter.

What's this?!It is really......I have to say,it is really cute!Like a baby's toy.We have a lifter, using two 2 Metre's rope and 2 safety hooks to create a lifting system. It can also slide on the main rope, but I can not imagine how you and your fellows put a truck's tire on a lifter,it is too heavy to carry. But,it is of creative!
