CINCINNATI - An investigation concludes that some very basic mistakes contributed to the deaths of two Ohio firefighters in a 2008 house fire.


The Colerain (KOHL'-rayn) Township fire department in suburban Cincinnati found that firefighters entered the home without water in their fire hose. Investigators also determined that the two who were killed got trapped in the basement when they tried to escape the flames without following the hose to find the way out.

Results of the review, being released Friday, were first reported by The Cincinnati Enquirer.

Recommendations from the investigation include the development of standard operating guidelines for the fire department and an increased focus on training and education.

While we're touching on this subject, does anyone have any data on what percentage of LODD's could be prevented by proper training, strict adherence to SOP/SOG's and every department adopting and STRICTLY enforcing physical readiness levels?

I'm guessing 80-90%?
