Does any one require that a request for a fire report to be a written request?
If so, what information is on that request?

We give the homeowner, the insured, insurance company, or on EMS reports, just the patient copies. This is only after the Chief has approved it. We have no written form. Keep safe and good luck John

only reason you would have a request form is for CYA (cover your Ass)..you really should do everything in triple anyway...but as a requirement thats on you guys ..I never heard of an official request form technically its public information unless it involves a criminal invest then it has to be court order subpena..
