More than likely the sign doesn't mean the defibrillator has been removed, only that if needed, campus security needs to be notified. Most likely their response is much faster than 911 EMS and they may have trained EMT's on shift. Typically these public defibs are set to alarm when removed.
Not neccessarily a FAIL. These boxes are a sold with three options.

1. The box simply holds the AED
2. The box is wired to set off a nuisance alarm when the door is opened. This is to deter stealing AED
3. The box can be sold with wiring option to tap into the facility FA and can set off a medical alarm.

The latter will get you a forced notification EMS response but it costs alot more than option 1 or 2. I suspect the picture above is to remind the user that they need to call campus safety, (on most colleges) who then dispatch EMS and Campus Security to the incident location.

And how do we know there wasn't a recall or some other issue with the unit?

Sorry some of these "caption this picture" stuff sure doesn't account for common sense, but a jump to conclusions in many ways.
