Hi Kevin,
If you re-read my post I didn't reference an article, it is a course I am taking that I spoke about. However, I came across a good article, about post-incident report writing, today while doing some homework: http://www.gordongraham.com/pdfs/GRIID_TRW.pdf

It is still not quite what I am looking for though. I am trying to develop a template of sorts so that a firefighter can quickly fill in the blanks with the appropriate inrmation while it's still fresh in their minds before leaving the hall to return home after a call. .
