Has your protocol changed due to increase in Rx drug use.

In our EMS district (which includes about 8 counties) on May 1 this year we added 2 more mgs of narcan to the drug box. This was in response to and increase of presciption drug use mostly in the form of fentynl transdermal patches being snorted, injected or placed in the mouth or in some cases the anus. We have also had increase of oxycontin which is increasing everywhere along with others. However the fentynl has been the biggest new thing we have seen. The interesting thing is that on these calls we seldom need the extra narcan we mostly need the coroner, instead. In a non related change we also added Zofran and removed cardiazem.

Due to the fact that we cannot get the non refrig. version. The kind we had was like glucagon where you have to pre mix. According to our EMS council the supplier was no longer providing it in that form. Also the council stated that the stats dont show us giving it enough to warrant the increased cost. So for good or bad we are not carrying it anymore. Of course we did this about 7-8 years ago then started carrying it again so who knows what will happen during next years reviews.
it is such a heavy narcotic in that state that it often takes at least 4 mg just to get to a state where they can reasonably control there own airway or breath for themselves. Thus the reason most are already dead before we can even intervene.
