List things that YOU have done wrong on the job and how you recovered from that mistake. Do not be shy as we are not going to make fun of you or tell you what you should have done. Just be open and honest so we can learn from the mistakes of others so we do not make the same mistake.
Lets see, I will start so people posting will not be too afraid to post.
I was transporting a patient in Iraq and I failed to wear my seatbelt. Small mistake but big issues. I was in a Helicopter HH-60G Pavehawk. Well, long story short, I was thrown around the helicopter and I was all dinged up with some lacerations from equipment.
While fighting a fire I forgot to wear gloves and I ended up with some small burns on my hands, its not a big deal but then again, it could have been the difference of having a hand aand not having a hand.
Let's see, In my former brigade... using a small attack mini-pumper. I was appointed as a OIC. Then there is school fire one block from our station. It was my first time for me in command. On scene smoke showing at 2nd floor windows then ran towards the second floor to size up and call for two lines of hoses. When I got there, a civilian (a student) is manning a hose from a buildings fire cabinet. Try to yell at him to come out, he couldn't ... he yell back... he can't see and he can't move. I yell back. to follow the hose and follow the light. So the guy made it. The smoke is starting to be unbearable then on my left almost right beside me I broke the window to vent. Another responding unit OIC came up and ran toward to my right in which perhaps I did not see it first.. he opens all the windows. The smoke subsided and able to knock down the fires. It turned out, I vented the wrong side in which the smoke can't vent in the hallway and should have known that there is some windows to my right in which it should vented outside the building. I found out later on, the school admin is not happy with the broken windows to my left. Good thing they didn't know who did it. Anyway, that's a learning experience for me.

i happen when i was still a rookie......, i was off duty on day, im on my way to work on board a public utility vehicle.... when a commotion caught my attension along the road. There was a visible smoke coming out of a restaurand., thinking it was a FIRE.... I immediately got my radio and call on for response..... i send a false feedback over the radio...... when the nature of the fire is a chimney fire..... it was not that serious to call for a POSITIVE ALARM which i did in the first place.....
After the incident, the radio dispatch advice me to verify wisely what is the situation before giving feedback over the radio.... Since that time.... i learned the proper way of doing scene size-up.... proper radio transmission of giving a feed back....
and the most important thing .........PRESENCE OF MIND DURING EMERGENCY SITUATIONS........
BREATH.....THINK.....BREATH........EVALUATE...... AND IMPLEMENT........ FEEDBACK...........
I was left by our fire truck on the fire scene after putting out the fire once in Makati City. Haahaha! I was chatting with some friends away from our truck, I didn't noticed that almost all the fire trucks in front of the tanker of our friend already left, including our pumper! The guys on the top of our truck thought that I was riding inside and the guys inside thought that I was riding on top of our truck! In the end, I hitch with another fire truck which is passing by near our HQ.
Lesson: Be sure to advise properly to your chief if ever you are going somewhere when the fire is out.
