Don't get tunnel vision it's a trap that can ruin yours and your crews lives. These things seem to happen at the wee hours of the morning when we are not quite on our A game. Sorry for the firefighters that where injured. Thankful no one was killed. Let's use this an an opportunity to learn.

No reason why this should have happened I dont care what and how many excuses you come up with, NO fire or emergency for that matter is worth getting hurt or killed over.... Slow down and pay attention to your response.....
